    Chloroquine torsades

It is a polymorphic ventricular tachycardia that exhibits distinct characteristics on the electrocardiogram (ECG). Symptoms and consequences include palpitations, dizziness, lightheadedness (during shorter episodes), fainting (during longer episodes), and sudden cardiac death. Torsades occurs as both an inherited (linked to at least 17 genes) Common causes for torsades de pointes include drug-induced QT prolongation and less often diarrhea, low serum magnesium, and low serum potassium or congenital long QT syndrome.

Erythromycin is a macrolide antibiotic that is widely used for various infections of the upper respiratory tract, skin, and soft tissue. Similar to other macrolides clarithromycin, azithromycin, erythromycin has been linked to QTc interval prolongation and torsade de pointes TdP arrhythmia. DRUGS TO BE AVOIDED BY CONGENITAL LONG QT PATIENTS CredibleMeds® has reviewed all available evidence and has placed the following drugs in designated risk categories for their risk of causing torsades de pointes TdP Known Risk of TdP KR, Possible Risk of TdP PR, Conditional Risk of TdP CR or Special Risk SR. Quinine, chloroquine, artesunate-amodiaquine and dihydroartemisinin-piperaquine, have been associated with a low risk of cardiotoxicity. 2. Drug-induced QT/QTc interval prolongation is a surrogate indicator for increased risk of drug-induced torsade de pointes TdP, a potentially lethal polymorphic ventricular tachycardia.

Certain drugs and combinations of drugs resulting in drug interactions are common contributors to torsades de pointes risk. It can be seen in malnourished individuals and chronic alcoholics, due to a deficiency in potassium and/or magnesium.

The aim of the study. This publication provides an overview of published clinical studies results on pharmacokinetic and/or pharmacodynamic drug-drug interactions in humans either healthy volunteers or patients and their electrophysiological consequences manifested as change in the QT/QTc interval. Hydroxychloroquine can be detected in the cord blood at delivery in concentrations similar to those in the maternal serum Costedoat-Chalumeau 2002. In animal reproduction studies with chloroquine, accumulation in fetal ocular tissues was observed and remained for several months following drug elimination from the rest of the body. The risk of torsades de pointes probably also varies with the type of drug irrespective of the effect on the QT interval. The absolute risk of torsades de pointes for sotalol and quinidine Kinidine appear higher than those associ-ated with amiodarone, although all three drugs cause marked QT pro-longation.
